Information
To enroll your child, please:
  1. Fill out the Pre-Enrollment Form and email to: office@azurehillspreschool.org
  2. Contact the center to check for availability: 909-825-7054.
  3. Complete the online application or pick up an application from our office.
The following documents, forms, and fees must be submitted before enrollment is complete:

Non-Refundable, Annual Registration Fee per Child
Original Birth Certificate
Immunization Card
